I'm having some problems with the latest git checkout of the TOC extension.

This:

### h3
### h3
#### h4
### h3

Becomes:

-h3
   -h3
     -h4
   -h3

All header are nested under the first header. I'm not sure if this is the intention, but it looks a bit weird.

After I reverted the changes from this commit: 93dddf9eb7b3263beda332e79ee84cf8ab9c13bb, everything behaved like it did before.
